Reviving old characters

A question was put to me from a past player. They are considering returning now and again but their character has undoubtedly been put in storage since it's been about 2-3 years. To whom do they request a retrieval and what information does that team member need?

Terry will need the GSID and Character Name.
An approximate date they last played would be helpful as well.
